You may be asked instead to fill in and sign a printed copy of the pre-course questionnaire before or at the start of the course. Upon receipt of your booking details from the Leadership Institute, the facilitator(s) of the course may contact you for a confidential pre-course interview.

Intentions of the Course
Each of our mindfulness courses is intended to help improve physical and mental well-being, personal effectiveness and a sense of quality of life. If you have experienced stress, overwhelm or anxiety, the course can help you develop practical skills to better respond to everyday challenges.
Non-clinical. Not a Treatment
It is NOT offered as a treatment for any physical or psychological condition. It is NOT suitable for people who are currently experiencing very severe issues physically or psychologically. However, you are welcome to attend the course at a time when learning is more suitable.
If you have simple health care needs, please inform us so we can try to accommodate them if we can. In general, please take responsibility for all aspects of your health during or after the programme as we are unable to do this for you.
If you are not sure if Mindfulness is for you at the moment, please check with your GP (or other clinical professionals). If you are currently experiencing feelings of distress, despair or suicidal thoughts, you may want to contact the Samaritans. They provide confidential emotional support 24 hours a day. Please visit www.samaritans.org.
Commitment Required for Learning
The course is highly experiential and will require about 30 to 45 minutes of home practice, 6 out of 7 days per week for all weeks of the programme. While there is sufficient amount of conceptual knowledge involved in the course, the majority of the learning is achieved only by practicing mindfulness. This is why course applicants need to be ready to commit the time and effort required for learning.
It is essential to have a computer or a smartphone that can stream or download the audio files for home practice assignments. Without these, it will be very difficult to learn the mindfulness practices spread throughout the course.
You may want to come in comfortable clothing that enables you to breathe easily and make gentle movements. For the first four weeks, you are invited to bring a fitness mat, a light blanket, and a towel to cushion your head when we do some of the exercises lying down. Sitting is also an option for the exercises if lying down is challenging for you.
For teaching quality purposes we occasionally video record our course sessions. We will direct the camera only at the facilitators but class discussions will be audible. The recordings are used for internal purposes only. Please feel free to speak with the teacher, if you have any concerns about this.
Data Protection and Privacy
When applying for a specific course, the information you provide will be kept securely, regarded as strictly confidential and destroyed 14 months after the course, unless stated otherwise and have gained your explicit consent. It will be used mainly by the facilitator/s for the purposes of knowing requirements you may have and if the course is suitable for you at this time.
You may be contacted by the facilitator to discuss if the course is appropriate for you to attend at this time. If you mutually decide that it is not, you may take the course at a more suitable time or condition. To prepare for the possibility of a pre-course interview, please read below the contents of the pre-course questionnaire.
Sponsoring organisation (if your organisation is our client)
The course will require some gentle movement. Please specify any limitations that may make it difficult for you to engage in gentle stretching, standing, sitting and walking.
Have you had any mental health issues such as anxiety or depression in the last few years? Please describe the issues as well as the professional support you are getting (if applicable).
If you had a difficult or disturbing life experience in the last year, please specify them. (e.g., bereavement, loss of job or home, major relationship breakdown, etc.)
I undertake to be fully responsible for my own health and well-being before, during, and after the course, and agree to bring any concerns to the course facilitators in the first instance, and contact my GP (Doctor) or relevant health professional, should I continue to have concerns about my health and well-being.
I give consent to the facilitator(s) to video record the sessions if necessary, for quality purposes, with the camera normally directed only onto the facilitator(s).
I agree to maintain the confidentiality of any information I gain about other participants during the course.
